AUTHORIZED AND ACCEPTED: You are hereby authorized to make the necessary specified re-pairs. I understand that payment in full will be due upon completion of the vehicle, including addition-al supplemental damage and charges. I hereby grant Aurora Body Shop LLC permission to operate the vehicle on streets, highways or elsewhere for the purpose of testing and/or inspection. If my vehicle is brought in with the fuel tank empty, I understand I will be responsible to reimburse Aurora Body Shop LLC for any fuel that needs to be added to the tank plus a $5 fee. An express mechanic’s lien is hereby acknowledged on the specific vehicle to secure the amount of repairs. Aurora Body Shop will not be held responsible for loss or damage to the vehicle or articles left in the vehicle, in any case that is beyond their control. Old parts removed from vehicles will be disposed of unless otherwise instruct-ed.
I authorize Aurora Body Shop LLC to disassemble my vehicle to assess the damage for repair. I under-stand that if my vehicle is disassembled, it is possible that Aurora Body Shop LLC may not be able to reinstall all of the damaged parts onto my vehicle. If a situation arises that I want my vehicle reas-sembled to the best of Aurora Body Shop LLC’s ability, without repairs being completed, I understand that I may be responsible for paying a fee for this service. I also understand that if I decide to not have my repairs completed at Aurora Body Shop LLC and parts have been ordered, I will be charged a 30% restocking fee at retail prices.
I authorize any and all payments to be made payable directly to Aurora Body Shop LLC. I am also aware that should an insurance check be sent directly to me, I am obligated to forward it to Aurora Body Shop LLC with my endorsement. I authorize Aurora Body Shop LLC to act with power of attorney to sign payments on my behalf should they be sent directly to them.
